/ nursing home/ day care center empanelled. You can procure proposal
cum offer document to be part of our network of Medical Service
Providers. Your facility to be empanelled by us should be --
* Registered as a Hospital /Nursing
Home /Pathlab with the local authorities and is under the supervision
of the registered and qualified medical practitioner.
* If Hospital /Nursing home, it
should comply with minimum criteria as under:-
1. It should have at least 15 inpatient beds.
2. It should have fully equipped Operation Theater of its
own wherever surgical operations are carried out.
3. Fully qualified nursing staff under its employment round
the clock
4. Fully qualified doctor should be in charge round the clock.
Below are the required documentation that needs to be completed
and submitted to the below address to complete the empanelling
process. Please read the instructions carefully and return the
same duly signed and stamped on all pages to the below address
or TTK Branch Office close to your facility.
Note: Empanelling into TTK's network does not necessarily
assure you 100% of business. It is an opportunity for your business
to grow but our intention is only to provide wider options for
the customer to choose from and gives you a benefit of servicing
over 4.3 million+ lives that is covered by our services.
